
| Health Insurance Initial Eligibility: 450 hours in any consecutive four month period, or 580 hours in any 12 consecutive-month period. Coverage begins on the 1st day of the 3rd month following the month satisfying the initial eligibility. Ongoing Eligibility: 140 hours per month are needed to maintain coverage. Hours worked in excess of 140 are banked for future shortages that may occur. Maximum allowable bank is 280 hours. Coverages: Provides hospital, medical, dental, vision, and prescription benefits. Short-term disability and life insurance coverage are also included. Preferred Provider Groups can be utilized for further savings. Non-utilization of a PPO member, may require various deductible limits must be met. Pension Currently, each member has the right to request a transfer or cash payout of their pension amount after leaving covered employment for the period of 18 months. This is a non-vesting program. |
